Hospital Social Work and Intermediate Care Service

Conwy Social Services and Conwy Intermediate Care Services provide a service for adults who are in Hospital or have recently been discharged from hospital. They can discuss personal matters and offer counselling, advice, help and information about care services.

What do we do?

The Conwy Social Services are responsible for offering assessments to people in hospital (and their carers,) to see if they may need social services after they leave hospital.

For acceptance to the Conwy Intermediate Care Service (CICS), there must be an element of re-enablement.

Hospital social workers do not deal with childcare. If you have concerns about childcare, phone the Children's Services on 01492 575111.

What is the aim of the service?

  • To promote and facilitate a safe and timely discharge from hospital.
  • To promote independence and enable adults to live in their own homes wherever possible.
  • To aid recovery and re-enablement.
  • To ensure a social work assessment is carried out under the NHS and Community Care Act 1990 and to be provided with services as a result.
  • To ensure carers are aware of their rights under the Carers Act 1995 and the Carers and Disabled Children Act 2000.

Who can I contact if I am not in hospital?

Hospital social workers work closely with community social workers. If you need help before going into or after leaving hospital, you can contact the Customer Care Team.
