Welcome to the Conwy Health Social Care & Well-being Partnership Website. The website has been developed to inform the general public, service users, carers, and staff in the public, voluntary and private sector in Conwy about the developments being made to improve the health and well being of the Conwy community.
The partnership includes representation from Service User & Carer groups, Conwy County Borough Council, North West Wales NHS Trust, Conwy & Denbighshire NHS Trust, Conwy Local Health Board, Welsh Ambulance NHS Trust, the National Public Health Service, Conwy Community Health Councils, Conwy Voluntary Services Council, the independent (private) sector, North Wales Police and North Wales Fire and Rescue Service.
At a strategic/planning level there is a Partnership Board and at an operational level there are Management and Policy Groups (MAPs).
Health, Social Care & Well-being is only one area where partnerships have been formed to improve services in Conwy. A Community Strategy for Conwy is in place which brings together all the different service providers (public, private and voluntary) with members of the community. The Community Strategy ensures that services meet the needs of the local community and therefore work towards raising the local quality of life. The Health Social Care & Well Being Partnership forms one element of the Community Strategy's action to improve the economic, social and environmental well being of the area and its people.
