The team consists of two full time Occupational Therapy Falls Instructors and one full time Falls Prevention Co-ordinator.
You can ask for support yourself, or can be referred by your doctor or social worker, or an exercise professional or voluntary sector worker.
After you have been referred to the service, a member of the team will carry out a personal falls risk assessment with you. They will then decide on the best course of action to reduce your risk of having a fall.
This may include one or more of the following interventions:
- Exercise classes: There is strong evidence that these can help improve balance, strength and confidence and have a positive social benefit too. This includes Postural Stability Instruction (PSI).*
- One-to-one community physiotherapy and individualised home exercise plan.
- Other activities in the community such as Tai Chi, over 60s gentle circuit classes, National Exercise Referral Scheme (NERS).*
- Home safety assessment and adaptations by Occupational Therapy or Care and Repair. • Medication review by GP or Pharmacist. • Referral to Podiatry or Optometry services.
*A small fee will be charged for some activities.
The North Wales Falls Prevention Programme is supported by partners from the NHS, local authority, and the third sector in your area, in hospital, in the community and in care homes. If you live in Conwy and have had a fall, or are fearful of falling, for more information contact:
