Commissioning in Social Care

As part of Social Care’s transformation programme a commissioning team was established to support the department. The aim is to develop outcome-based commissioning that place individuals at the heart of everything we do.

Our Approach

Our approach to delivering integrated health and social care services focuses on individuals within their communities and families. It emphasises responsibility and control by citizen with services facilitating and supporting that role – the self-caring citizen appropriately supported when required. Central to our service vision is the role as the individual as the expert, taking responsibility for, and at the centre of planning future support and care. Services will be focused on developing and maintaining individuals’ independence through supporting the development of strong community networks and services that support people at home. 

The commissioning team are currently working jointly with the other North Wales local authorities and health colleagues on the North Wales Population Needs Assessment which was recently approved and published. This information is informing our Conwy Social Care commissioning strategy and commissioning plans for each service.
